Conditions Upon Acceptance

If your application is approved, please note that any scholarship or financial discount provided by AZ EVO ELITE is conditional. To continue receiving this support, you (and the awarded player) must:

1) Consistently demonstrate behavior and conduct that align with AZ EVO ELITE standards.

2) Ensure all payments are made on time. Any failed payments or past-due balances must be resolved within one week.

3) Notify AZ EVO ELITE Program Director when financial assistance is no longer needed.

4) Attend all practices and or Events/Fundraisers (unless a emergency situation arises, please notify the Program Director in a timely manner.)

***All applications that are accepted are for a 2 month period and if situation stays the same, a new application will need to be filled out.***
